Steps:
- Check all screens.
- Search using Spotlight – to check if the app is still on iPhone.
If you find it in one of the screens:
- Press the icon until it wiggles and drag it to where you would like it to be.
Else
Reset the home screen:
- Settings | General | Reset Home Screen Layout.
